If your 5th - 12th grade student is planning on participating in any sport at Noble Academy this year (5th graders can participate in basketball and cross country in the spring), he/she needs to have a medical release signed by a physician. This medical release serves to assure you as the parent and us as the school that your child is healthy and able to participate in sports. Without a signed medical release, your child will not be able to participate in any sporting event, including practice.

Most doctor's offices provide this service, and you are welcome to have it done on your own. In order to make things more convenient for our families, we have arranged for Dr. Enrico Jones (father of 7th grader Adam) of Friendly Urgent and Family Care to come to Noble Academy for this purpose.

Mark your calendars for Tuesday, August 31 - Dr. Jones will be on campus starting at 3:30. If you wish for your child to have a medical release done by Dr. Jones, please fill out and return the front page of the form along with a $40.00 check made out to Friendly Urgent and Family Care.
